Care Management Indicator Utility

Create a Care Management Indicator Dashboard to Track Patient Participation in Care Management Programs

Having completed several successful data projects with Keena over the years, this large multi-specialty health system in the western US asked Keena consultants to create a patient care dashboard that could provide access to essential care management program data directly through Altera TouchWorks..

Keena quickly went to work to create interfaces between CarePort Manager and Care Director within the Altera TouchWorks EHR. Keena then collaborated with the organization’s clinical and IT teams to create the UI for the dashboard. The result was a user-friendly console and query tool that the entire care management team could easily modify and expand as needed.

SPRING TIP

ColorGuard orders Exact Sciences
Create custom reports for repeated printouts. Gain efficiencies by automating fill-in of discrete data.

Current manual workflow:

  1. Staff place Cologuard order on the patient in TWEHR
  2. Staff fill out a specific Cologuard form with patient/order details
  3. Staff manually fax the form to Exact Sciences

Optimization w/ custom Crystal Report:

  1. Staff place a Cologuard order on the patient in TWEHR all the rest is done automatically.

One-time Configuration:

  • Additional printout (crystal report) is created and added to TWEHR as a new custom printout
  • Auto-print defaults are setup that anytime an order is placed with the RPL (Requested Performing Location) of Exact Sciences, a fax is automatically sent being the form required by Exact Sciences
